Patient and Family Centred Care

Patient and Family Centred Care (PFCC) is an approach to the planning, delivery and evaluation of healthcare that is grounded in mutually beneficial partnerships among health care providers, patients, and families.

The Patient and Family Centred Care office is committed to the development of authentic partnerships from the bedside to the boardroom with patients, families, and staff to improve the quality, safety and experience of care.

The Patient and Family Partner Role

A Patient and Family Partner is an individual who has received care or supported a family member through care who can offer insights and input to help provide care and services that are based on patient and family identified needs.

Patient and Family Partners partner with staff in all endeavors; from working with staff to improve processes, sharing their stories, to being a member of a working group.

The four core concepts of Patient and Family Centred Care are:

  1. Respect and Dignity
  2. Information Sharing
  3. Participating
  4. Collaboration
