If you were born between 1978 and 1994 your childhood immunization records may still be a hard copy record. If you were born before 1978, your childhood immunization records are unavailable. 

In order to provide you with the most recent copy of your immunization record, Public Health searches for your record in the electronic provincial immunization registry called Panorama.  If a hard copy record is also located, it will be back entered into this system. Records are stored in this registry if you have lived or attended school in Saskatchewan under the Saskatchewan Health Authority.

  1. If you were born and raised outside the Saskatoon area and are wanting a copy of your immunization records, please contact your local public health office from your city/town.
  2. If you were born and raised within the Saskatoon area and are wanting a copy of your immunization records or if you have a MySaskEhealthRecord Account and don't see your full immunization history, please choose from one of the options below:
  • If you are an adult who would like a copy of your full immunization record from Public Health, are over the age of 16 years, there is an administrative fee of $25. Please complete the Immunization Record Request-Adult form.
  • If your are a parent/guardian wanting a copy of your child/children's immunization record and they are 16 years of age or younger, please fill out the Immunization Record Request -Child -16 and Under form. There is no charge for children 16 years and under..

Please send your completed request form(s) in by fax 306-655-4711 or email phsrecordline@saskatoonhealthregion.ca to the Saskatoon Public Health Record Line. 

Currently the options to receive an immunization record are by fax or mail only.

A record of immunization from Panorama will be provided for clients, physicians, and public health offices within 7 business days of the client contacting the Saskatoon Public Health Record Line.

Personal health information shall not be released to any other person other than the individual or a parent/guardian of children under 18 years. A parent/guardian cannot request an immunization record for a person 18 years of age and older without written consent being provided to Public health.

Personal health information, including immunization histories can be mailed or faxed to the client. Personal health information that is faxed is sent with a confidential notice, identifying sender on the fax cover sheet and intended recipient and number.

Clients may give permission verbally, in writing or by email for someone else to pick the record up.
